Collaborative Community-Based Care Management Programs
Durham Community Health Network
A Community Care of NC Program
When clients sign up for Medicaid through Carolina Access, they have the option of choosing the benefits of the Community Care of North Carolina Program, administered locally in Durham through the Durham Community Health Network (DCHN). At the time they enroll, clients may choose a primary care provider who will manage their overall healthcare needs. If they need specialty care, the provider arranges for the client to see a specialist or receive other health services, such as nutrition counseling. DCHN is designed to help people access the healthcare support they need when they need it. DCHN works hand in hand with the client's health care provider to ensure that they receive the care recommended by their provider. Read more about this program
